Patient: 11, Name: Unknown, Age:approximately 34, Sex: Male
Patient 11 suffers from Retrograde Amnesia believed to be caused by past traumatic experience. No medical history is known. Showing signs of Source Amnesia an explicit memory disorder, he is beginning to recall certain information, but not where or how it was obtained. He is under the Schizophrenia and Bipolar Disorders program in Adult Inpatient Hospitalization, he has not received the final diagnosis of Schizophrenia and is only suspected at this time. He is currently being treated with psychiatric supervision and medication management, sedation when necessary, group therapy, individual psychotherapy, art and music therapy.
I am filing this report to document an accident that occurred on February 14, 2012 at approximately 3:00pm. Patient 11 was in group art therapy when he crafted out of a cardboard a set of what appear to be angel wings and a wire hanger halo he insisted on wearing these throughout the day. When the head psychiatrist arrived at the hospital, he suggested the craft was not in favor of Patient 11’s recovery. When hospital staff attempted to remove the craft from Patient 11 he refused and became physically aggressive towards staff. Patient’s 11 stated “I need them, because without them I can’t go back to him, without them I cannot make it up to him. Without them he will never forgive me.” When the Patient 11’s private psychologist arrived on the scene he was asked to explain who he was talking about for the privilege of keeping the craft. Patient 11 hesitated and when he was unable to clarify on his statement he became aggressive towards his private psychologist and had to be sedated. The craft was removed from Patient 11, and he has not brought the incident up neither in group therapy or in private psychotherapy.
